(NOTE: Team names will be based on current NBA and college teams. Only teams with all players completely registered – three items turned in – will have first come first pick on team names. Check the bottom of the site to see which names have been taken.)
ALL PLAYERS MUST TURN IN ANY PROOF OF AGE
(Proof of Age Includes: Birth Certificate, State I.D. Card, End of the year report card)
**If any of the three items are missing on July 18, 2018 by 4:00pm, player will not be eligible to play in tournament**

- Parent Release Form
- Proof of Age
